AMENDMENT 00001 7 SEP 2026 All EAL request for the site visit must be submitted to the CO and CS listed in the contract information. 55 CONS will require the following information for each person wishing to attend the site vist: Legal Name of Guest: (Last, First, MI) Sex: Male (M) Female (F) DOB: (YYYYMMDD) Driver�s License# & State or FNV Passport# or Immigration ID#: Citizenship (Country): Replacement and removal of an un-level multi-surface flooring in the TACFIT area of the Offutt AFB Fieldhouse. SITE VISIT A site visit will take place on Monday, 14 September at 1300 CST. An EAL form will need to be requested from the POCs listed on this solicitation. Contractors interested in attending the site visit will need to complete the EAL form and return it to the POCs listed on this solicitation by COB 7 September 2026. Base access is not guaranteed.
